Care professionals can access to the web platform as soon as they are registered by their center coordinator and receive their credentials by email. Once they can enter the web platform, they can register older adults.
Older adults start their SOFI journey when their care professional register them and receive an exclusive invitation, guiding them through the setup.
Firstly, older adults must complete a comprehensive frailty assessment designed to understand their specific needs. Our initial questionnaires are crafted to tailor SOFI’s programs directly to older adults, ensuring they receive the most relevant and beneficial care resources.
Once the frailty assessment is done, older adults can benefit from a personalized physical activity program, a tailored nutritional plan, educational content and many more features. In parallel, professionals can follow-up older adults through the web platform and make informed decisions.
